Local Thai plan or international plan for an expat?
Where you would be treated decides it. The insurers say, in their own words, where each plan pays.

HelloPrakan, a Thai health insurance comparison site, answers “local Thai plan or international plan?” with one question back (where would you be treated?), because a Thailand plan is written for treatment in Thailand and pays abroad only for emergencies on a trip, while an area-of-cover plan is written for treatment across a region or the world and is priced for it. The insurers’ own territory wording for each plan on our panel is below, with the visa rules that decide it for some buyers.
What 'local' and 'international' mean on the documents
The words are ours; the insurers use their own. A Thailand plan states its territory as Thailand with an extension for trips; an area-of-cover plan asks you to choose a territory and prices it. The two are listed on different compare pages here (Thailand, Regional and International) and never in one ranked list, because the cheaper plan would usually be the one covering the smaller territory.
Thailand plans: what they pay abroad
| Insurer · plan | What the document says |
|---|---|
| AXA Insurance (Thailand)SmartCare Essential | “This Policy will cover the Covered Person within Thailand and will extend to cover the Covered Person while traveling anywhere abroad ... not exceeding 90 (ninety) days any one trip”, for “injury from an accident or immediate and unforeseeable illness ... requiring medical treatment as an inpatient”. |
| Allianz AyudhyaSmarter Health Plus | “This insurance plan provides coverage for medical treatment occurring in Thailand only.” |
| Allianz AyudhyaSuperior Health Plus | “Benefits for Non-Inpatient Care, will be provided in Thailand only.” |
| Pacific CrossMaxima / Maxima Plus / Ultima | Main coverage area Thailand; out-of-area coverage 90 days worldwide per trip. |
The pattern: cover in Thailand, with an emergency extension for trips of a stated length, for in-patient treatment, and never for travel made in order to be treated. If you expect to be treated outside Thailand by choice, these plans are not written for that.
Area-of-cover plans: what they pay, and where
| Insurer · plan | What the document says |
|---|---|
| AXA Insurance (Thailand)SwitchCare | “Choose Your Area of Cover in Asia or Worldwide excluding USA”; “Emergency Treatment Outside Area of Cover (30 days per trip)”. |
| AXA Insurance (Thailand)IE | “Cover for pre-existing conditions” as a stated benefit, with a “Pre-existing Conditions Benefit” by policy year and a 270-day figure printed beside it. |
| Allianz AyudhyaBeyond Care | “Worldwide coverage excluding USA”, where “the benefits will be paid for the injury from an accident only”; haemodialysis and organ transplantation covered in Thailand only. |
| April InternationalMyHEALTH Thailand | “Area of Cover Options”; outside it, cover up to ฿1,637,500 per period of insurance for sudden illness or injury in the first 30 travel days of a trip. |
Three things these documents make explicit that a “worldwide” label hides. The USA is excluded or limited to accidents on every area-of-cover plan above. Some treatments are tied to Thailand even on a worldwide plan: Allianz Beyond Care pays haemodialysis and organ transplantation in Thailand only. And treatment outside the chosen area is covered for emergencies within a stated window: 30 days per trip on AXA SwitchCare, the first 30 travel days of a trip on April.
These plans also differ from one another in structure, so they are compared within structure, not as one list: how to compare plans.
Pre-existing conditions: the one place the type of plan matters most
Every Thailand plan we hold a document for excludes pre-existing conditions. Allianz prints it as “Chronic diseases, injury or sickness (including complications), conditions or abnormalities that have occurred before entering the insurance contract (Pre-existing Condition)”. Two area-of-cover plans on our panel handle them differently. AXA IE prints “Cover for pre-existing conditions” as a benefit, with a per-year “Pre-existing Conditions Benefit” and a 270-day figure beside it; April offers moratorium underwriting, under which a condition treated in the 24 months before the start date is excluded but “once you have completed a 24-month period where none of these apply, the medical condition may then be covered”, with a stated list of conditions that may never be. If you have a condition, read those clauses before you read a price.
When the visa decides for you
For the Non-Immigrant O-X, TGIA states that “All applicants must contact authorized insurance companies directly to apply for Thai health insurance policy”, with not less than 400,000 baht in-patient and 40,000 baht out-patient cover per policy year. For the Non-Immigrant O-A, a policy “either from Thai or from Overseas insurance companies” is accepted, with minimum cover of USD 100,000 accounted as 3 million baht, and an overseas policy needs the Foreign Insurance Certificate signed by the insurer. AXA sells EasyCare Visa, which its brochure says is “designed to comply with Long Stay Visa (O-A and O-X) and Long Term Resident Visa (LTR) requirements”. The visa guide has both rules in full.
How to decide
- Where would you be treated for something serious: Thailand, or elsewhere by choice?
- Do you hold or want an O-X visa? Then the policy must be Thai.
- Do you have a pre-existing condition? Then the IE and moratorium clauses above matter more than territory.
- Then compare within the group you have chosen, at your age, from the insurers’ own rate tables.
Local or international: common questions
Should an expat in Thailand buy a local Thai plan or an international plan?
Buy for where you would be treated. If that is Thailand, a Thailand plan is written for it and pays abroad only for emergencies on a trip, within a stated number of days. If you want to be treated across a region or worldwide (to go home for treatment, for instance), the area-of-cover plans on our panel (AXA SwitchCare and IE, Allianz Beyond Care, April MyHEALTH Thailand) are written for that and are a separate group with their own structures and prices. HelloPrakan never ranks the two groups against each other.
Does a local Thai health plan cover me when I travel?
For emergencies on a trip, within limits the insurer states. AXA SmartCare Essential extends cover abroad for trips of up to 90 days, for accident or immediate and unforeseeable illness needing in-patient treatment; Pacific Cross states 90 days worldwide per trip outside the main area. It does not cover travel made for treatment, and Allianz Smarter Health Plus covers treatment in Thailand only.
Does the Thai visa require a local or an international policy?
The O-X visa requires a Thai policy: TGIA states that applicants must apply to authorised insurance companies for a Thai health insurance policy. The O-A accepts a Thai or overseas insurer, but an overseas policy needs TGIA's Foreign Insurance Certificate signed by the insurer.
Which international plan for Thailand is best?
We do not name one. The area-of-cover plans on our panel differ in structure: AXA SwitchCare has an annual structure with a chosen area, April MyHEALTH Thailand an overall annual limit per period of insurance, Allianz Beyond Care a per-confinement maximum, so they are compared within structure, at your age, from a live quote.
What we could not source
These are things readers ask about that we could not confirm against an insurer’s or regulator’s own document on the date this page was written. We leave them out rather than guess.
- Territory wording for Chubb Samaggi, Bangkok Insurance, Viriyah, Thai Vivat and Thai Health plans: the brochures we hold either do not print a territory clause on the pages we could read, or (Thai Health) are image-only. Their plan pages show what the engine holds.
- The Baht amounts of AXA IE's pre-existing conditions benefit by year, and whether the 270 days printed beside it is a waiting period for that benefit. The brochure prints the figure; the English wording we hold does not, and the Thai wording is the governing text.
- Anything about Cigna Global, Luma or other insurers not on our panel.
- Which type costs more. Premiums come from each insurer's rate table through a live quote; a general statement would not be a number we could defend.
